What families can expect.
We begin with your child’s story, symptoms, timeline, prior care, and the questions your family most wants answered. The care team may review imaging and reports, complete an age-appropriate examination, explain the finding, and talk through the next step.
Arrange access to the actual imaging files before the visit whenever possible. Actual imaging, not only a written report, is required for surgical review. Bring the written reports, prior clinic or operative notes, medication list, and insurance information. A familiar comfort item can also help a child feel more at home.

You do not need to have every answer before you call. Our team can help you understand what to gather for your child’s visit.
Tell us how we can help
Use the appointment request form or call 754-300-0817. The team must confirm your appointment; submitting the form does not reserve a visit time.
Confirm the visit details
Ask the team to confirm your appointment location, insurance participation, any referral or authorization requirements, and how to complete new-patient forms.
Arrange your records
Gather your child’s imaging files and written reports, prior consultation or operative notes, and current medication list. Ask the team how to send records securely before the visit. Do not upload medical records or enter medical details in the general contact form.

Where do I complete new-patient forms?
Call the practice for current intake instructions and a secure way to complete your forms.
What if I do not have the images yet?
Tell the team when requesting your visit. They can explain which records to request and how to arrange secure delivery.
Can we ask for extra support at the visit?
Let the team know about your child’s communication, sensory, mobility, or other support needs when booking. You can ask for explanations to be repeated and take time to write down your questions.
